31 Mar Reimagining Dementia Care: Lessons from Implementing the GUIDE Model
The Health Care Transformation Task Force and MedStar Health released a case study featuring MedStar’s implementation of the Guiding an Improved Dementia Experience (GUIDE) Model, a federal program designed to strengthen care coordination and caregiver support for people living with dementia.
Through the GUIDE Model, MedStar receives a monthly payment per beneficiary to support care coordination and implementation, and a one-time infrastructure payment to offset start-up costs. These resources enable MedStar to deliver interdisciplinary, person-centered dementia care that addresses both medical and non-medical drivers of health, while supporting caregivers as essential members of the care team.
MedStar’s approach serves as a framework for how similar programs can improve outcomes across a wide range of high-needs populations.
