A 60-year-old man with multiple chronic conditions was tired of emergency hospitalizations. He had missed many of his primary care appointments and stopped taking his medications, leading to several hospitalizations. After his last hospital stay, he was contacted by a nurse, who worked with him to better manage his health.

His care team helped him to choose a care plan that matched his wants and needs, which included remaining in the comfort of his own home. He decided on hospice care so he could continue spending time with his family and friends.

Because of #ValueBasedCare, he is now pain-free, has stayed out of the hospital, and is enjoying time with loved ones.

After losing her husband and facing serious health and financial struggles, a 36-year-old mother of three found hope and healing through her care team. 

With worsening hearing loss, depression, and anxiety, she felt overwhelmed until her provider connected her with a nurse practitioner (NP) who listened. The NP supported her with accessing group therapy, support with daily needs, and a renewed sense of strength. This is #CareThatMatters. 

“The NP cared, understood and listened to me. It changed my dynamic. I wish everyone could experience the care I had. This was a blessing in disguise. They really gave me life. I feel way better than ever.”

Peter is a Medicare patient living with coronary artery disease and congestive heart failure. Peter struggled for years to manage his health, until #ValueBasedCare made personalized, proactive support possible.

During one of his virtual check-ins, Peter’s care manager noticed a concerning symptom that he hadn’t recognized. Peter was able to schedule a same-day visit to the clinic, to get a timely diagnosis, effective treatment, and a help to afford his medication.
