23 Jun The Task Force Releases a Case Study Featuring Strive Health’s Kidney Care Model
WASHINGTON, D.C. (June 23, 2026) – The Health Care Transformation Task Force released a new publication, A Better Model for Kidney Care: Earlier Identification, Smarter Coordination, Better Outcomes, featuring Strive Health’s work to improve outcomes for people with chronic kidney disease and end-stage kidney disease.
Strive Health’s multidisciplinary, value-based kidney care model demonstrates that earlier intervention, whole-person coordinated care, and financial accountability are associated with improved outcomes, including a 41% reduction in hospitalizations, 11% fewer ED visits, a 62% increase in home dialysis, and up to a 20% reduction in total cost of care.
