The Health Care Transformation Task Force released a new case study featuring Strive Health’s value-based kidney care model, highlighting how risk-aligned partnerships and multidisciplinary care can improve outcomes for people with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) while lowering total cost of care.
Strive’s model has produced meaningful quality improvements, including a 41% reduction in hospitalizations, a 20% reduction in total cost of care, and a 94% patient satisfaction rating.
