Webinar: Engaging Consumers in Care Delivery

The Health Care Transformation Task Force released an implementation framework for engaging consumers in care delivery to promote ideal person-centered, value-driven health care by including patients and consumers as partners in all aspect of decision-making about their health care.

The Implementation Framework provides best practices and details case studies of high-performing programs around three core elements of care: coordination of care and systems of care, including physical/mental health and social services; shared and empowered decision-making; and individual activation for self-management. This webinar provided insight into how organizations can use the framework to evaluate existing efforts and support new initiatives. Representatives Community Health Plan of Washington and Partners HealthCare shared how their organizations are putting these person-centered standards into practice.
