WASHINGTON (February 26, 2025) – The Health Care Transformation Task Force (Task Force) – a group of leading health care payers, providers, purchasers, patient organizations, and value transformation partners – released a new case study in collaboration with Chamber. The publication, Driving Value-Based Cardiology Care at Scale, highlights Chamber’s care strategy, financial arrangements, and outcomes. This publication coincides with American Heart Month, recognizing the importance of advancing high-quality, prevention-focused cardiovascular care.
Chamber’s model provides cardiologists with real-time data, shared workflows, and flexible financial arrangements – including capitation – across Medicare Advantage and other risk-bearing entities.
