31 Mar The Task Force Releases a Case Study Featuring MedStar Health’s Implementation of the GUIDE Model
WASHINGTON (March 31, 2025) – The Health Care Transformation Task Force (Task Force) – a group of leading health care payers, providers, purchasers, patient organizations, and value transformation partners – released a new case study in collaboration with MedStar Health: Reimagining Dementia Care: Lessons from Implementing the GUIDE Model. The Guiding an Improved Dementia Experience (GUIDE) Model is a federal initiative designed to improve care coordination and caregiver support for people living with dementia.
MedStar’s program supports dementia patients and their caregivers to help patients remain safely at home for longer. MedStar delivers interdisciplinary, person-centered dementia care that addresses both medical needs and non-medical drivers of health. Through the GUIDE Model, MedStar receives a monthly payment per beneficiary to support care coordination and implementation, and a one-time infrastructure payment to offset start-up costs. MedStar’s intervention serves as a framework for how similar programs can improve outcomes across a wide range of high-needs populations.
